At Coastal Spine and Pain Center, Dr. Formoso is able to provide our patients with the latest and most effective treatment solutions for the treatment of acute and chronic pain.

Dr. Formoso is a Board-Certified Physical Medicine & Rehabilitation specialist.  Dr. Formoso earned his bachelor's degree in Psychology at University at Buffalo in 1996, and earned his doctorate degree at the New York College of Osteopathic Medicine in 2001.  He is a graduate of the Temple University/MossRehab PM&R residency program where he was nominated Chief Resident, and a graduate of the Interventional Pain Medicine fellowship at Temple University.

Located at two locations - Jacksonville Beach and Middleburg - our offices are well-equipped with state-of-the-art technology that enables us to perform diagnostic testing and various interventional pain management procedures right on the premises.

The goal of our practice is to provide the most advanced and effective treatment for patients with spinal disorders and other painful conditions in a compassionate and caring manner on an individual basis.
